This notice describes the information you provide through the public Dreams to Doorstep inquiry form.

What you share

The form collects your name, email, participation interest, and any optional phone number, location, organization, and message you provide. Please do not submit financial documents, account numbers, Social Security numbers, or other sensitive financial information here.

How it is used

Your inquiry is saved for the D2D Program Director to review and follow up about your interest, program fit, and next steps. It does not create an account, submit a mortgage application, or enroll you in the program. The public form does not display your submission to other visitors.

Technology and access

D2D uses Vercel to host this website and Supabase to store inquiries. These providers may process technical information needed to operate and secure the service. The inquiry inbox is restricted to authorized D2D program directors. Participation in the Command Center involves separate account and role-based access.

Your choices

Optional fields may be left blank. You may contact us to ask about your inquiry, correct information, request deletion, or tell us not to follow up. Some records may need to be retained to meet legal or operational obligations.
